It comes from the Hyper Text Coffee Pot Control Protocol, an April Fools document that has outlived most serious specifications. It is not part of HTTP and no client does anything meaningful with it.
Developers use it as a deliberate signal: a placeholder in a stub endpoint, a response to obvious bot traffic, or an easter egg. Because it is unmistakably intentional, a 418 in a log is usually someone leaving a note rather than a fault.
