When you see it

A slow or interrupted upload, a client that opened a connection and did not send anything, or a network that stalled mid-request. Note that this is the request being too slow, not the server being too slow to answer, which is 504.

Some servers send 408 routinely to close pre-opened idle connections. Seeing occasional 408s in logs with no user complaints is usually normal housekeeping rather than a fault.
